Contact a support professional (Addiction Counsellor, Social Worker, Primary Care Provider, etc.) to complete a referral to House of Friendship’s Live In Treatment Programs. Self-referrals will not be accepted to these programs.
Important Information for Referring Professionals
We do not accept referrals for individuals seeking to transition to treatment directly from incarceration. Please contact Connex 1-866-531-2600 for programs that can support the needs of these individuals.
Pease fill out all sections of the external referral form. If a field is not applicable, please write N/A. Do not leave any fields empty as it may delay processing of the referral.
Referring professionals can fill in the fields on the external referral form and make changes over multiple sessions simply by clicking “Save” after each session. Once you click save, a Referral Link and a Password will be visible at the top of the form. Copy this down somewhere secure as you will need this information to access the form at a future time.
Once the referral form is complete, press submit and our Intake team will process the referral. No changes can be made to the form once it has been submitted.
